Personal Accident Insurance Coverage of up to ₱50,000*

This coverage includes:

  • Loss of life due to an accident
  • Accidental injuries causing permanent disability
  • Accidental injuries resulting in being permanently bedridden
  • PRIMA members aged 0 to 99** at the time of enrollment

How to Activate your PRIMA Access by MaxiHealth

After your purchase, you need to activate your PRIMA Access eCard with the Reference Code before you can avail the benefits.

*The membership registration period is up to three (3) months from the date of purchase.

Are PRIMA Prepaid Cards Covered by Insurance?
PRIMA cards activated on Jan 15, 2026 or later are automatically enrolled in Personal Accident Insurance at no extra cost. Coverage includes loss of life due to an accident, as well as accidental injuries resulting in permanent disability or being permanently bedridden.
What is the Validity Period of the Insurance Coverage?

The insurance coverage begins upon card activation and ends on the expiration date indicated in the Welcome Letter.

Who is Eligible for the Built-in Insurance?

All PRIMA members who activated their cards starting January 15, 2026 and are aged 0 to 69 years old at the time of enrollment are eligible for this built-in insurance coverage.

Starting June 15, 2026, coverage will be extended to members aged 0 to 99 years old.

What is the Life Insurance benefit and what does it cover?

The Life Insurance benefit is provided through Aspire Care, a voluntary top-up plan from Maxicare Insurance that offers supplementary coverage for death, accidents, or critical illness. Benefits include:

  • Basic Life: PHP 50,000 for loss of life.
  • Accidental Death: Additional PHP 25,000 if death is due to an accident.
  • Critical Illness: PHP 25,000 lump sum for Major Cancer, Major Stroke, or Heart Attack.
  • Terminal Illness: 50% advance of the Basic Life benefit if diagnosed with a terminal illness.
Who is eligible for the Life Insurance benefit, and when does the coverage begin and end?

The Life Insurance benefit is available to PRIMA Consult, Consult+, Access, and Elite members aged 18 to 65 who activate their membership on or after July 1, 2026. Coverage begins upon receipt of the PRIMA card activation email and ends on the date indicated in the PRIMA Welcome Letter.

How do I file a Life Insurance claim?
To file a claim, submit the required documents via email to claims@maxilife.com.ph. Soft copies may initially be submitted for evaluation; however, original hard copies must be received before any claim payment can be released.

For Teleconsult:

      No appointment is needed. You may simply call our hotline anytime at (02) 8582 1980 or (02) 7798 7798 to be immediately connected with a doctor for your consultation.

For HomeLab Service:
      The HomeLab add-on may only be purchased once and is valid for one-time use only. Once included in your plan, you may schedule your HomeLab visit via the link sent to your registered email.

Note:

  • A referral to a specialist is required and will only be issued after an initial consultation with a General Practitioner (GP) or Internal Medicine (IM) doctor. However, members may also see a specialist if they present a valid outside doctor referral.
  • Laboratory tests can be scheduled but must be endorsed by a Maxicare Clinic doctor. Tests outside the plan coverage will be out-of-pocket. Members may also undergo laboratory tests if they present a valid outside doctor’s request
How can members avail of medicine discounts through MedGrocer?
Members can order via the ForwardMG website using a valid membership code and prescription. Orders are processed within one working day and delivered within 1–5 days depending on location.

The program covers 150+ subsidized medicines, subject to formulary availability.

What does the Gen Zen Mental Health Wellness Program include?

Members gain access to counseling, diagnostic support, wellness services, and specialist recommendations through enrollment on the Gen Zen website.

Ongoing follow-ups and co-managed care are provided based on clinical need and assessment. 

Gen Zen Mental Health Wellness may be availed as early as the following working day. 

How can I avail HomeLab under PRIMA Access?
HomeLab is available as a top-up add-on and must be purchased together with PRIMA Access. The HomeLab top-up may only be purchased once and is valid for one-time use only. Once included in your plan, you may schedule your HomeLab visit via the link sent to your registered email.
